AGREEMENT FOR PROFESSIONAL INVESTIGATIVE SERVICES

I / We, the undersigned (hereafter known as Client), hereby agree to employ the services of Eagle Eye Detective Agency & Associates (hereafter known as Eagle Eye), a private investigative agency, duly licensed and bonded under the laws of the State of North Carolina, solely for the purposes of attempting to:

Client agrees and understands he/she shall be solely responsible for the compensation of said investigative agency at the hourly rate of $___________, per investigator, plus out of pocket expenses (i.e. photos, videos, sources, et cetera) and mileage at the rate of $_______ per mile, per vehicle, as well as documentation fees arising from said investigation. The giving of depositions and court ordered testimony shall be considered part of the investigation and payable at the same above stated rate. Client understands and agrees that Eagle Eye will not begin the investigation until said retainer has been paid. ^{Client’s Initials: ______}

The retainer of $____________ is due immediately upon Client signing this Agreement. Client acknowledges Eagle Eye’s investigative fees and expenses incurred, which are not covered by said retainer, will be due immediately upon receipt of each invoice, but no later than 14-days after receipt of each invoice. Eagle Eye reserves the right to impose the following late fees and interest: to Client’s unpaid invoice(s): Up to $500.00 past due, $25.00 late fee plus 5% interest; over $500.00 past due, $50.00 late fee plus 7% interest. ^{Client’s Initials: _______}

In the event Client defaults in payment of sums due to Eagle Eye, and if the Agreement is placed in the hands of an attorney at law for collection, Client will be responsible to pay all costs associated with the collection of said unpaid funds, including, but not limited to a reasonable attorney's fees, plus Eagle Eye’s fees and interest. ^{Client’s Initials: _______}

In consideration of the foregoing terms and conditions, Client understands that Eagle Eye shall under its best efforts investigate the matter set forth above on behalf of Client. However, Eagle Eye cannot guarantee the outcome of said investigation.

Client hereby agrees Eagle Eye will conduct the investigation on my behalf at its sole discretion via any lawful means it deems appropriate.

I, the undersigned, my heirs, beneficiaries, devisees, legatees, administrators and assigns further agree to indemnify and hold harmless Eagle Eye and/or its agents and employees from any and all actions, causes of actions, claims, damages and demands of whatever type wherever situated arising directly or indirectly from this investigation, which Client has requested above

If any portion of this agreement is held to be invalid, then the remainder shall still retain its full force and effect.
